Medical Social Work Services
Whether you have been recently diagnosed or have a chronic illness, the Social Worker is an integral part of the health care team. The Social Worker provides what is called psychosocial support. This is a broad term covering the personal, emotional, financial and social issues that may affect a patient and family during illness. Following a skilled assessment, a care plan is developed to work toward the strengths of the patient and family during this period of medical care.
Social work services are confidential and available to home care and hospice patients, their families and caregivers. All Visiting Nurse Service & Hospice of Suffolk’s Social Workers are certified with a master’s degree from a school accredited by the Council on Social Work Education.
The care plan may include the following services:
